Kora for care teams

The discharge packet, finished. The family, not lost.

For discharge planners, clinics, and the doctors who say yes to the recording. The family keeps one plan they can find, and your office can see what they were told, when the family says so.

The packet exists. Nobody can find it when it matters.

Your patient leaves with papers. The family leaves confused. Your office gets the phone calls: which pill was stopped, when the follow-up is, what the doctor said about the swelling.

What the family already has

  • Working today

The discharge papers, photographed, read and filed into a living plan the whole family can see.

  • Working today

Follow-up visits recorded only after the doctor's spoken yes.

  • Working today

A plain-words debrief of each visit, sixty seconds after it ends.

The family's debriefExample

Mom's follow-up with Dr. Rivera

Monday, sixty seconds after the visit

What the doctor said
Her blood pressure is lower than last time. Keep the new evening dose.
What changed
Stop the old water pill. The pharmacy has the new list.
What happens next
Blood test in two weeks. Call the clinic if her ankles swell again.
Example. Dr. Rivera and the family are made-up. A debrief carries what the doctor said, in plain words.

What your office gets

A read-only view, granted by the family, of the visit debrief and the discharge packet. A nurse line or a clinic can see what the family was told and what they are working from. The family grants it, the family can take it back, and your office sees nothing else in their circle.

  • Working today

A family can already send you one visit's debrief as a read-only link. It expires after thirty days, and the family can turn it off sooner.

  • Pilot scope

A standing view for your office, of the debriefs and the discharge packet together, is set up with clinics in the pilot.

The discharge hand-off: one link

At discharge, the planner hands the family one link. The family opens it, photographs the packet, and has a plan they can find at 11 at night. There is nothing for the hospital to buy, install or connect.

Pilot scope The link and a printed card made for your unit are set up with you in the pilot.

The hospital discharge checklist we give families

A visit is recorded only when the doctor says yes, out loud, first. Illinois law requires everyone's consent, and Kora will not start without it. The recording is deleted after it becomes the family's written summary.

How we treat your family's records

  • Your records stay in your family's circle. Only the people you invite can see them.
  • A visit is recorded only when the doctor says yes, out loud, first. Illinois law requires everyone's consent, and Kora will not start without it.
  • The recording is deleted after it becomes your written summary.
  • Your family's information is never sold. Ever.
  • Nothing about your family is used to train models unless you say yes. The default is no. You can export or delete everything, anytime.
